We need to gather some basic information before your first visit. Please complete the patient registration forms using the link below. If you need us to obtain medical records from a previous physician, please also download the medical records release form.

Prescription Information:

When you need a refill on a medication, please contact our office at least 48 hours in advance. This gives Dr. Powers time to review your child’s chart to determine if a refill is appropriate or if your child needs to come in for a follow-up or lab work, etc.

When calling, have the following information available:

• Your pharmacy’s name and phone number
• The name of the medication(s) you need refilled

As a general rule, we do not call in medication refills after hours or on weekends except in emergency situations.

Hours:

Our office is open Tuesdays, Thursdays, and Fridays from 9AM - 5PM.
